The Social Services Department in Sarah Neuman /Westchester Division of The New Jewish Home is seeking for a full time Social Worker to join their team! 

Job Responsibilities:

• Work as part of an interdisciplinary team to provide comprehensive discharge planning, provide emotional support to patients/residents and families, complete MDS and psychosocial assessments.

• Coordinate and facilitate discharges, care planning and completion of all documentation

• Facilitate team and family meetings, education on resident rights and well-being needs of the vulnerable elderly and ill population..

Skills & Requirements

• MSW from an accredited School of Social Work. 
• Valid New York State License in Social Work.
• Experience and/or expressed interest in working with adult and geriatric population.
• Experience in long term care preferable , solid written, communication and organizational skills
